Any tips for someone with ADHD playing League?

Hey League Community.

I'm Silver 5 and I play ranked a lot. I carried myself from Bronze 5 to Silver but now I'm stuck. I can't carry myself anymore and most of the time I feed late game because I have ADHD. I can't concentrate on anything but what's happening in front of me. Map awareness is very hard for me, because I can barely concentrate on one thing as it is, let alone 2 things.

I also forget when to go into teamfights and make very stupid mistakes often that lose us the game. I feel so useless because all my friends excel more than me at the game, even some who have been playing for a less amount of time than me.

League is also pretty much the only game I play. I hate sports with a passion (also because of ADHD) and can't stand console games because they're often rushed and buggy, plus most of my friends are long distance. And I'm not trying to insinuate I don't have a life, because I spend time with a few of my IRL friends every weekend. League is my only source of fun with my far away friends though, so that's the point of making this thread.

I really just need tips or something to help me concentrate. I refuse to be prescribed medication for personal reasons, so don't say that. If anyone would be willing to give me general tips on how to be better at the game then I would appreciate it ever so greatly. Right now, it's super painful to even try to play League in my elo because of the stress and my inability to concentrate on anything.

I aim to get to Plat by the end of this season. Not gonna give up.
