If say both teams kill each others nexus at the same exact time hows it tell who wins? Is that like possible, has it ever happened before?

I don't think it'd ever happen. The game dictates who finishes first, even by the millisecond. So say Ekko hits the nexus .001 seconds before Tryndamere last hits the other nexus, the Ekko would win. But say they DID hit at the EXACT same moment, i think it'd just crash the game and go 50/50 on who would win. But who knows! (Riot probably does)

Its the most recent one. If your team surrenders but then suddenly the enemy team surrenders about 1 sec after you did, the screen will start moving towards THEIR nexus, and you will get a victory. For last hitting the nexus, its the most recent one as well, even if you ended it 0.3 secs before the enemy did.
