That's clearly as face of something. Any thoughts on what it could be?

I see how you can think it's a face however i'm 100% sure her splash art is just to show her blowing things up. Think about it she has a gun that shoots cannon balls and she tosses bombs on champions and structures. I'm pretty sure this just shows that she blew up something. I mean this could also say she released something when she blew whatever that is up never know.
That giant fortress is actually Noxus.
Seeing as Bandle city is allied with Demacia, this makes sense.

Ehh, the "eye" on the right (closest to Tristana) has a faint, but still visible, smoke trail that indicated where it was launched from. The mouth seems to just be more fire, and the other eye is possibly fire that's being blown by the wind. But it is a good catch! ^.^ You never know, it may have formed that way by some external force other than Tristana.
