Take a break when you lose ranked. Trust me, the eagerness to go back after a loss just to earn that LP back is tempting, but you'll most likely lose if you do.

My chart looks pretty similar. I wouldn't say I'm tilted though, I just keep getting absolutely terrible teammates lately.
Take a couple days off of League. Go for a walk, hang out with a friend, watch a movie, go on a date. Just get off of League for a couple days.
Once those couple days are up, come back. Playing during the day or middle of the night is the best option for climbing, do whichever is more accessible while still being good. (Don't play late at night if you normally go to bed by 11pm. Basically, don't play past your normal bedtime by more than 1 game.)
The times stated for playing, if possible, are the best for avoiding ragers/flamers/feeders/trolls, as more often than not, they're going to be in school during those times. Weekends and any time school isn't in session (summer/spring/winter break) are AWFUL. Stick to norms during those times.
Do anywhere from one to three warm-up games before going in to ranked, EVERY TIME. The warm-up game will get your juices flowing, will get you all lubed up and ready to go. If you tilt during this game, take a break. Get up, stretch, grab a glass of water, maybe a sandwich, and come back. Wait at least 5 minutes.
Basically, what this'll do is get you untilted and have a fresh mind, league wise. You'll be set to go, in the best mindset possible at that time, and you'll have a higher chance for winning.
If you want proof of this working, check out my op.gg (EllenTheGenerous). I got seeded Bronze 5, got to Bronze 1 within the first week after provisionals, then got to Silver 5 a couple weeks later (I got super busy after that week, haven't played as much.) I'm currently Silver 1, almost Gold 5. Last season, my peak rank was Silver 3.
Lastly, bronze and silver are a MINDSET. By removing any sort of mindset at all (my only mindset is that I'm playing league. there's no strings, my team doesn't suck ass, im not the savior of the game, im not better or worse than anyone. im just playing.) you will quickly climb out because you'll be able to point out your own mistakes easier.
