Like it says in the tool tip, it heals off of literally everything, including true damage, spell damage, on-hits, and AAs.

Have you tried Nashor's on Diana? If so, then you should try a hybrid bruiser build with Nashor's, Gunblade, and Guinsoo's as your core. You proc your passive a ton, deal a huge amount of on-hit damage, and Guinsoo's+Gunblade is basically a better version of Ravenous Hydra.

As the patch notes say, it works of literally everything. Lichbane, Nashor's, Teemo's E, Rageblade, Thornmail, everything. You heal off everything.

Yes. It works on literally all damage you do, even true damage.
