mundo's a pretty good duelist. he has very high base damages and on top of that gets very tanky and hard to kill.

I just said Mundo is not as bad a duelist as you're making him out to be. His existence is to tank an entire enemy team and deal good damage to a single, squishy champion, as well as have some nice utility.

mundo has 6000 hp and vayne autos him, that's 120 bork damage she'll heal 38 from botrk

Even with reduction, he still reduces incoming physical damage by 4/7, or 64.3%. 3/7 of 8% is still only 2.9%.

Even on a 5k HP mundo, the healing from BotRK is rather low, as BotRK does not give much Lifesteal itself: The first auto on full HP Mundo will deal a maximum of 400 damage from BotRK's passive. That's 40 HP regenerated.

Chances are Mundo has 150 Armor even after LW, giving that first attack 160 damage and 16 HP regenerated.

In all honesty, the effect sounds more impressive than it really is. Unless you build additional Lifesteal, which also has its drawbacks:

A BT in place of a MR item means you won't outheal Thornmail, if that was your plan. And a BT in place of IE means that you might outheals Thorns, but you do less damage.
