Banning Champs and Nerfs

I've only been playing since 2012, but in that time I've only seen one champ addressed due to ban rate, which was Kassadin.

Is banning champs consistently something that flags a champ for review? Or is that wishful thinking, and will this change with a potentially 4 extra bans?

its hit or miss with riot, Take lee sin and yasuo for example both consistently stay above 49% win rate through nerfs and all both barley see anything happen they JUST now start balancing both champs and this has been a problem for awhile now thats on top of being the highest played in each of there respective roles
