What is your Perma Ban list for Ranked?

So we all have a set of champions we perma ban because of personal beliefs, we also alter it every patch for things change. So for patch 6.13 what has been your perma ban list?

My number 1

This may surprise some people, but the reason I find her to be op is for i'm in silver-gold elo. She is easy to play, she is strong, and has a game changing ult that was recently buffed. You are more likely to see a champion who is strong and easy to play in low elo vs a champion who is OP but hard to play.

My number 2 If someone bans Annie for me, I tend to lean toward ban Hecarium, super early pressure jungler, strong ganks, and great mid game, and since the ghost changes, just a pain to deal with.

My number 3 A good Fizz player makes the life of your mid laner a living hell, so if I don't have faith in my mid laner, I will often ban this guy. He has been strong for many patches with very minimum nerfs.
