5 bans weren't out when camille was out. If they were, Camille would be 100+ banrate.

Season 2.

Kassadin. Morgana and Ahri tended to alternate, or if one was banned the other was guaranteed ban as well, and usually 2nd or 3rd bans were always reserved for them if the other team didn't. Those three were at one point 100% ban rate every game for a good portion of a season.

I thought Zac and Yasuo (in low elo) were both really high ban rates when the 5 ban system was first introduced. I know Zac at least pulled ~90% in high elo at the height of his power this season.
