I do believe his q should not prioritize enemies if it can be spammed at 3 minutes. Instead of his ult increasing by 5 mana, maybe it should increase by like 20-30, so that Swain can not just "press R" to win.

Treat him like a juggernaut... but he's ranged and has a ranged slow... and he often goes top lane...
That being said, Swain isn't broken, but personally I find him very strong and very annoying to deal with, as well as it being an incredibly snowbally lane. If he gets ahead he'll kill you every time you walk into range, and he'll be standing behind your casters. If not, you'll either proceed to dump him every time your ult comes off cooldown, or you'll be kited and 'outplayed'.
