Sometimes I am winning 6 in a row... then I lose 3 in a row. Then I win like 2-3 games while also losing some games. Sometimes I am carrying hard and sometimes I am the reason we lose. I am not flaming my mates and I am capable of playing all roles.
How can I reach platinum? What can I improve?

You are playing well...just need more games. If you keep playing like that you will get Platinum. But the season resets in 2 days anyway..so whats the point?
Pick a main role and trim down your champion pool, if you have close to an even w/l ratio on a champion rethink whether they are worth playing, if not swap them out for another champ, if they are, dedicate more time to learning to play them. you have a few champs with a 60%+ win rate, if you can keep that up then playing just those will allow you to climb.
