I'll get straight to it.

Twitch R: Plague / Sewage Bolt 100/90/80 second cooldown.

Twitch's next auto attack gains +450 range. ( This auto attack has 1000 range. ) The auto attack hits the first champion struck, fully stacks "Deadly Venom" and increases the damage of "Deadly Venom" on this target by 100/200/300% for 6 seconds. Target spreads "Deadly Venom" to close by allies. ( 42 True Damage per second at level 6, 90 True Damage per second at level 17. Maximum possible true damage: 540 )

Again, concept. That true damage would probably get nerfed.

As a twitch main I strongly dislike this suggestion because his q and r are really what define him, this change would give him more of a instant effect and not a period of power, which would definitively not be as fun to use, this also would make playing him more risky, he doesn't have any dodge or dash and would quite easily be killed once going for a follow up after his r.
