What causes matchmaking to place you with good/bad teammates?

I noticed lately that I've been matched with really bad players; what exactly determines in matchmaking who you get matched with? Sometimes I get matched with great players. Sometimes its more of a level field of some good and some bad in one team... Sometimes I get matched with a group of penalties (people who have been banned in the past for bad behavior). What determines this?
