Finally got out of Bronze.
After much frustration, and complaining, many on these very boards, I've finally gotten out of Bronze and just wanted to help other still struggling to get out.
The main thing I did was watch this guy. He puts out a video every day showing how to carry low elos. Watch his videos every chance you get and eventually his lessons will slowing be ingrained in your mind and your play will improve. Now you can't watch one or two and call it quits. You have to watch them over and over again, every time you have a chance, even if you watch some of the same ones repeatedly and you must listen to what he is saying. He does a great job of explaining what and why he does what he does. Listen enough times, and his knowledge will soak in and you will find yourself playing a lot better. You have to be playing as well. Don't just watch. Watch, listen and play.
But let me give you the short of it, that is, the main lessons that helped me climb:-
The first and most important thing is CS. Good CSing is the number one thing that will help you climb. You should aim for 80 by 10 minutes. Now you're not gonna be getting that every game but your margin is 80 by 13 minutes. If you cannot make 80 CS by 13 minutes, don't even think about getting out of Bronze until you fix this. This alone is enough to get you from B5 to B3. You can carry 80% of B5 games by good CSing.
The next thing is understanding level and item advantages. In general, all things being equal, you should not try to fight someone 2 levels ahead of you. In most cases you will lose. Respect level differences when deciding if you're going to all in. This is one of the main reason Bronze players feed out the ass. They don't understand this. Item advantages, the same applies. If you're laning against someone and they finish their first big item before you, then in all likelihood, they will win any 1v1 duel. This even applies to smaller items. If after a skirmish your opponent backs and comes back to lane with

, your opponent has the item advantage. Pay attention to your opponents' items when laning against them. This will get you fed and prevent you from feeding.
The last point I want to make is not one I learned from the videos, nonetheless it helped me. Main one or two champions. I got out using nothing but Kayle Mid/Top. Now this tip is not essential but it will be much faster than playing multiple champions. Every champion is a world of its own and even the simplest champions have complex ways to play them. The more you play a single champion, the better you will get at them. Play them enough and you get a huge advantage from this because chances are you're going to be playing vs someone who is playing a champ that they don't know nearly as well as you know yours which will help you to win lane and carry games.
Anyways, there's more I can add but I think this kinda sums up the main things needed to get to Silver. GL.
UPDATE: 4/13/2017
I was totally completely wrong. What I failed to realize at the time I wrote this was that I had accumulated well over 1000 ranked games when I reached there from Bronze 5 . While yes, the stuff I mentioned above does help, all it does is helps you to NOT be the reason your team loses. The reason you win any match is entirely dependent on your team.
So how did I make it to Silver last season ? The MMR system the match maker uses in League is designed to give each player a roughly 50% win rate. You can overcome this by consistently not being the reason your team loses. That means don't feed or otherwise do anything to throw your games and you will carve out a slightly above 50% win rate. What this also means is that you have to play a lot and I mean a lot of games for this to work in your favor. You'd have to be extremely lucky to climb from Bronze 5 to Silver 5 in less than 100 games. The point is, you as a single player have zero control over whether you win or lose. There are too many random factors that determine the outcome of a game for a single player to overcome. After 3 seasons and thousands of ranked games played I've finally realized that.
I started this new season, season 7 at B5 and peaked at B1 after 900 games. I'm currently on the verge of being demoted to B4. I have no doubt I'd make it to Silver again but I'm guessing that will take about 300 games or so. Last season I made it after 1300 or so ranked games.
So the the short version is this: If you want to climb, try not to be the reason your team loses and play lots of ladder matches. You should climb a tier in about 800 to 1500 ranked games.
