Why can I see the animation and hear the sound but people can still get out of it at that point if they flash or I die?

I ulted Gangplank and he flashed over a wall, heard the sound, saw the animation but the Ult did not go off. Not on cooldown either so it's not like it hit but didn't kill him.
Romans VI XXIII1 votesGarens ult can be canceled if you go into the Fog-of-War during the animation. You have to have vision of your target the during the entire animation or else it gets canceled. SO if GP flashed over a wall, you lost vision of him, and thus canceling the rest of the ult.
