Question to midlaners:How you deal with lack of sustain in lane ?

I was a malz main for past several seasons, and I use 6% spell vamp runes before. After the spellvamp rework, gunblade is the only item that gives sustain from ap dmg.(ROA's sustain does not have much to do with dmg or your ap) But not every mid champ is good with gunblade. So I use this kind of rune not only as malz, but also sometimes when I feel the matchup will be rough and I want to stay in lane longer. And also this sustain may help during late game when you got low and don't want base because of important objectives. Combines with last season's mastries it was 8% spellvamp which is a good amount. I know not so many midlaners doing this, so people have their way to deal with this kind of problem. But for me this season is a huge change, I will likely to struggle for a bit, especially as malz. So I want some tips or advice.

I honestly just play safe and only go for trades I feel have a high percentage chance for success. You could also grab Ravenous Hunter in the Domination tree, that is stacking spellvamp on takedowns if you want it. It's 2.5% and 2.5% more for each stack. You can gain up to 15% through that if you get your 5 stacks so it's not too bad.

You don't, why do you think AD mid laners are common place? I miss will of the ancients, basically a bloodthirster for mages.

It's about managing your hp and choosing your trades. Never take free damage because that can easily mean an incoming gank.

If I've learnt something is that rough matchups are won if you can farm well.

I just play like a little B!tch.

I focus a lot on dodging skillshots. If I feel like the enemy is about to engage I retreat a bit. I know that I farm better than most people I play against so why bother trying to kill them if it puts me at risk? Just farm up and kill them later.

Try domination as second tree (it has a spell amp option if I recall correctly) or inspiration with its biscuits (even though that works only until minute 12).. Meanwhile I am in pain for mana sustain.. Manaflow excludes ultimate hat :/

play safe, mages shouldn't build sustain at all, gunblade is a waste on malzahar if you ask me, when you play malz mid take tp and flash, this way you can get back fast if you really need to. use the minions and voidlings strategically (time it wisely) for dps damage, you can bully your opponent away from the waves, or even reach them with dps damage.
