What can I do as Swain if my team is behind?

Honestly, as fun as Swain is when he's got his items, there feels way too long of a period where my team can make it difficult for me to keep up. This is especially bad when I'm going top lane as a counter pick -- even if I grab a few kills, there's not much I can do without my core build items.

So, should I just keep farming and pushing or do I group as soon as I can? Do I let them get CS or should I take it and try to carry?

Swain's carry potential falls off late game. He doesn't have the best burst damage as it's all DoT, and instead reverts to a tank. If you get fed early game, he feels godly, as you are doing all types of damage.

If you have just a few kills going for you, do your best to help the other lanes. Since you have sustain, you can take the brunt of most of the attacks while letting your team-mates get the kills.

If you get fed, try to end the game as early as possible. Get fed more, and push objectives.

At what point would you say teleporting to lanes other than my own is a better choice?

If Swain is best as a tank, why don't people just build him without a ROA? If he falls off, it seems like a waste to buy instead of getting something to help his power spikes.

Because ROA gives you a ton of HP, Mana, and some AP. Since you have a ton of DoT you can pick it up and maybe a rylai's and simply outlast people while you tick them down.

There is no champ that can carry a game with just a few kills to my knowledge.

It all comes down to global gold and XP, you have to win your lane, then help out mid and bots. Also, avoid stealing everything, it's hard when you are fed, but if you die for whatever reason, your team won't be able to keep up.

bump for help, I'd really like to be able to enjoy Swain in solo Q when it's possible.

Usually once you have dominated to the point you have taken your turret in lane, it's time to move on and help. Otherwise it's just keeping your eyes open ont he map and take advantages of mistakes when they happen.

RoA gives him health, mana, and damage. The more mana he has, the longer he can last with his ult. The higher his AP, the more damage his ult does, which in return gives him more life. So you see it is the best tank item for Swain, and it's pretty much a necessity build for him.

Alright, what would you say is an optimal build? I often take ROA, Zhonya's and Visage as my rush items. Are Rylai's/Liandry combos the most effective build afterwards? Are there times when it's better to simply go mostly tank? Should I build a lot of AP if I'm very ahead?

Nothing. A single QSS will always cut your damage by more than half.

If the rest of your teammates are just feeding, well the game is pretty much over. Your best bet is to just afk farm/push as hard as you can until it forces the enemy teams attention on you. Also if you have TP, you can use it to make TP plays around the map, just hopefully your other lanes ward so you have something to teleport to. In swains case I always felt he was more of a lane dominant champion who scaled well into mid game, but then falls off a bit later, so push your advantages as hard as you can early on.
