Replace item 3031 by item 3812 or item 3078 , Hell either you will do huge burst on Q or you will heal for all the damage you will deal. I tried Death dance Garen, it synergize more than you think ! Damage reduction, healing on Q and E and a bleed effect that doesn't remove perserverance passive , 10% cdr and 75 AD, that's huge with Garen scaling.
