In the same way that they hard coded Shyvana dragon form Q from proccing tiamat/titanic 13000 times when she uses it.
Because they realized that a champ could have a broken interaction with an item, and hard coded it out of league before it could happen.
They just did it with Ezreal double stacking tear items, so its not something that they havent done in a while either

Debatable. Rageblade is vaynes first item, because she can Q auto (or auto Q) for a proc that deals bonus damage with a high AD ratio.
Vaynes silver bolts should be like Yi's Wuju style... Or at least it should be on attack instead of on hit, so vayne has to actually attack three times.

definitely not her first item, but my point was she built rageblade for a long time without being broken with very few other changes.
