He is overtuned right now, they are already testing nerfs for him on the PBE.

And yes, he has a bad gameplay design (not to mention his visuals), that's pretty much why he is tier 1 for a VGU. There were just some points in his way for now.... like that they said they want to make a break from Zaun, since they already reworked Warwick and Urgot... And I can imagine that Riot has a hard time coming up with something for Mundo.

The only iconic parts of his kit are his cleavers and his ult. Unfortunately, his ultimate is really problematic as well, since it is so extremely sensitive for changes. It gets heavily reduced by Grievious Wounds, but if you buff it too much, he becomes an unstoppable beast that slaughters your entire team. Is it too weak, then he is not able to do anything in the game.

It would be great if we as the community could actually provide suggestions for what his "gimmick" could be that you have to work around. A new regeneration mechanic, that gives the enemy something to play around with, but that let's Dr. Mundo have some play making potential as well. And it shouldn't be so sensitive for Grievious Wounds.....

And maybe they want to preserve his niche as the "anti-AP-juggernaut".

I've always found that a first item null magic mantle pretty much destroys his damage early making you able to pretty much fight him freely(cuz his Q deals magic damage). I haven't played much PBE or faced Mundo on live in a long time though. That's not to say you should take Qs to the face, but it'll make his Qs hurt a lot less early game allowing you to beat him as long as you don't just get hit by every Q he throws. Still not happy about Mundo becoming good.
