What is an under powered / over powered champion?

Now before you go on saying Mordekaiser or Urgot , I was talking about what makes one.

I mean, when the balance team pulls out a good change, do they base the changes off how strong a champion is in the 3 phases (lane, mid and late) or is it to match the meta of champions and items being brought in.

I personally think most changes are based off of current meta since all phase strong phase champions are being spat out left (Yasuo) right (Fizz) and center (Jinx) and not being changed (Except Darius)

(note: I know there are more strong all phase champions)

Only a few select people in the entire universe can play zed to his full potential

At the time when Zed was extremely oppressive it didn't take a master to dominate a game with him.

Meta makes a difference but really it is trade offs of their strengths vs weaknesses. Is their laning phase super strong but they are also late game monsters or is it really hard for what you get? A "healthish" champion is Nasus. He has a hard time laning vs top lane bullies but if he is able to get 500+ stacks he will laugh at them as he smacks them around. People considered Rengar bad because his pre 6 ganks are lackluster and his clears are meh at best, but when he gets to late game good luck being an adc. On the other side people see lee Sin as balanced because he falls off later game if you cant use his kit full.
