Where/how can I improve?

This season I hit diamond (Yay!) but am afraid to try and progress because I feel like I'll fall flat.

I want to improve but I finally reached the point where I can't see my own mistakes any more. Most of the time whatever lane I'm in goes even or I get a CS lead and I roam aggressively.

I'm not a 1 trick. I have a vast pool of over 40 champs I can play (No ad carries) and I win ranked games on top, mid, jg, sup. I'm primarily a JG/mid.

I'd like to get better as I've improved from G, to P2, to D5. I'd really like to continue this trend and improve to at least D3 for next season.
