Change it to true damage and/or make it apply healing reduction on the attacker.

No. Thornmail is not an item designed to deal damage, but to negate the healing of ADCs. (it's hard to put into words).

It's designed to make that 60 healing from AAs nonexistent against Malphite because he has a item 3075 and now the ADC is also taking 60 damage. Not for an ADC to attack a tank and die for no good reason.

A 2350 gold item makes an adc buy a 3700g item and people still think thornmail sucks.
