At least in my opinion. Let him blink back and heal a bit (that makes sense with his story/character). This champ just seems too oppressive.

Oh, for fucks sake, stop quoting the cinematic as canon. The game isn't balanced around the god damn cinematic.

OT: Remove the damage off Ekko's ult will literally give him no reason to use it offensively, solving no frustrations and removing interesting gameplay.

If anything, thematically speaking the heal should go away, since it cleraly demonstrates that the damage and fatigue he has stays the same.

That is true, I forgot that he was still hurt in the movie. Truthfully I think losing the heal would balance it as well.
