Intentionally pissing off your teammates while refusing to communicate about and agree on strategy is bannable. Making a strategic decision intended to help your team is not. Without e.g. "screw this supp i'm going mid whether you like it or not" in chat, it's kind of hard to discern the two.

In theory its reportable... But mid is kinda blameable as well since he didn't swap. I doubt that he gets a warning (that may turn into ban) for changing position unless he was abusive or intentionally dying.

It's debatable and each case will be evaluated individually. With the information you provided it would be too hard to make a generalization what IFS or player support would do for that particular game.
