do not even bother buying for 3 reasons:
1.its price
2.they have to build hydra because they do not have a good aoe ability to clear the waves/camps
3.its way too dificult to stack out of lane.
Riot had the right idea with an older version of 
,where on its last stack in gave a hydra like effect.

seeing how well titanic works with ragebalde, that's not really a big issue
It's price paired with no defense whatsoever is a reason for sure, especially since you'd want atleast one other on-hit item to really make use of the phantom-hit. It also doesn#t give any stickiness. A bigger issue is getting the stacks up and useing them for melees outside lane.
Another issue were nerfs/reworks. Xin got his AP ratio on W nearly cut in half back in the guinsoo's + devourer days, yi got the ao ratio on meditation gutted, WW no longer is a really AA-focused champ, it's mostly his Qs, Irelia was never that great with it as her Ap-ratios a slightly lacking.
Rainfall1 votesthe item is too expensive and like i said earlier hydra and trinity or jungle item have a higher priority for those champions,by that point most of them start building tank so rageblade is never bought.
The entire point of that item was to replace hydra in those champions kits.that effect got removed by riot in season 5.

then tanks items.with titanic hydra and your W you have 2 autoattack resets which is good burst damage considering the amount of defensive stats you got.
