Did anyone else get this Survey from Riot two weeks ago?

Around May 20th, I got an Email from Riot that asked me to take a Survey. The Survey asked questions about Dynamic Que and Ranked, all about the validity of Rank and enjoyment of Ranked, with a few other things to.

Here's the link to it, though I don't know if it will work for anyone else, because I've already taken it. If it is possible for others to click into it, feel free to look at all the questions and discuss it. If it isn't then this Thread is useless, I guess?
