I am a terrible player

Let me start off by saying I'm a really bad player. The title isn't a joke. My mechanics are shoddy, my decision making on a macro level is ok but my micro (In fight) decision making is horrible, and I tilt easily.

I knew I wouldn't be able to fix most of these so I've done my best to work around them. I play mechanically simple but powerful champions like nunu, renekton, xin, and shyvana. I ward about 3x as much as the average often having more wards than the support or anyone else in game often buying sightstone, even if I cant make the right play someone else can off vision. I play early game champs so that if we lose and I tilt there was less of a chance of a comeback anyway.

My warding is literally the only positive aspect of my gameplay on blue baron but it has also led to my highest win rate ranked champ in nunu.

Does anyone else do things like this to shore up weaknesses or should I just work on my weaknesses instead of working around them?

Understanding your weaknesses and playing to minimize them is a good thing but you wont really improve much by doing it. I would suggest perhaps doing things like that for ranked games but then also weave in normal games or teambuilder games and force yourself to try and practice and work on the things you know you are weak at. It is especially helpful if you got a high elo friend that you can do scrims against and have them point out things you are doing wrong and give suggestions on improving them.

I play almost exclusively against diamonds and high plats in normal games and team builder. Our ranked team consistently plays against people in the top 500 on their champs.

Our all gold (With one person who just got plat 5) team scrims high elo teams on our TS server sometimes and we do well but when you are playing with people who are just better we try to work around it with lane swaps, vision and quick rotations because in standard lanes we'll get stomped.

at least you admit it, and at least you know what you gotta work on. get friends to play with you, or find some that are better than you that can give you tips and play against you in 1v1s to help you better your positioning and mechanics

Junglers cannot 1v1. I watch the LCS and pay careful attention to pathing of the junglers and where and why they chose specific lanes. I've always been a meh laner but I counter pick hard because I can beat master level hecarim mains with renekton since renekton counters hecarim and I can beat diamond 1 irelias who get constant jungle attention with hiemer because heimer hard counters irelia. I counter pick to make up for lack of lane skill but I'm primarily a jungler which means I can only learn in real games.
