It's probably in their builds, but the matches are ending before they finish whatever core things they are thinking "I need to get this before making the executioner. I'll make that next."

That said people don't have perfect memories. They don't see the champions you'd want to build it against in every match, so once they do, they might just forget about Grievous Wounds altogether.

It's irritating that I have to give up a tank item for either item 3165 or item 3123 .

Is irritating for any player to have to forego a core item or at least delay a core item because of the chanpion's they're against

Yeah it's really annoying like it's only 800 gold for FUCKING ANTI HEAL but basically every adc will just be like "I NEED MORE DAMAGE" even though all there damage will be healed.

HA HA HA never maybe when i get my last whisper shit am i even building a last whisper item? :/ eh fuck it, build guide doesn't mention it. oh oh wait it does eh well the item looks shit anyway, not buying it. lel. its also funny that its actually more then 15 ad (goldwise ofc) because of grievous wounds increases that math of 800 gold worth vs healers due to the fact your killing off bonus hp from that healing sources. you could say your getting extra gold worth from the passive in itself. the frustration of others who dont see that is very concerning. im glad you see its value though.

It’s bc it doesn’t build into a good item for champions other then Assassins. Mages have morellos, tanks have thornmail, and Assassins have mortal reminder. Bruisers don’t have a good anti heal option, supports don’t need one because they should be focused on healing and cc not anti heal, and adcs don’t need one because late game they do enough damage that healing doesn't matter. Bruisers still need a good anti heal option because the are the top laners and are the ones generally going against the sustain champs.
