First thing I see is too much focus on kills not enough on farm. Your CS scores are SO LOW. Try aiming for 50+ CS every ten minutes. Also keep in mind escape routes. Just because you see a low health squishy DOES NOT mean go in. Make sure you can make it out after going in. A lot of your games your kills and deaths are about equal which tells me you are taking too many risks to get a kill. Focus on NOT DYING. Learn your kill zones, how much health you can remove at level 3, 6, and 11 and only all in when you KNOW they will die and you will live.
If you feel like your mechanics are polished for your tier, my guess it's the stuff that's happening between the kills that's the problem.
Make sure you play around vision. I'd say you should pretty much always have a pink ward on you. All the mechanics in the world won't save you if you unknowingly dive into 4 people when you thought it was gonna be just the 1. Abuse your mobility and the fact that few people can 1v1 you. If you see the enemy ADC farming jungle but all of the surrounding brush are unwarded...you don't have to dive in there for the kill. You can just sit in the side lane and free farm, the ADC will have to look for scraps up until you get the wave pushed to tower. I believe like 18-20 cs is roughly equal to a kill these days, so just pressuring him off the wave for a while can have strong payoffs. But don't sit in the lane after it's pushed up. You slow push it, looking for opportunities for kills, and if none arise you just get the wave to tower for pressure/reset and be happy with the 6-8 cs advantage you just forced him off of.
Objectives, objectives, objectives. Kills for the sake of kills is very, very rarely a win condition. It's only the most snowbally of games that it works out that way. So start working on 1 primary habit. Every single time you secure a kill or your win a skirmish, stop yourself from chasing for more kills, and look at the map and ask yourself what can I take off that? 50% of a tower hp? A buff? The bigger prizes are obviously things like herald and drake, but even stealing away a single jungle camp before backing can impact a game. Remember for junglers they have to go to the monsters, they don't come to them. And once laning starts to break down the rest of their team is gonna be poaching. So if you take away a couple camps and then the jungler wastes their time walking over to take Gromp only to find it's not there, 10 seconds of wasted time and 1 camp of farm might seem little to us, but suuuuper annoying and potentially impactful for a jungler.
Before a game even starts try to understand your win condition. Too often assassin players in solo q have a 1 track mind. They literally only roam around hunting kills, or they spend the entire games in side lanes refusing to do anything besides split push. As soon as champ select is completed look at their team comp and look at your team comp and try to lock an "ideal" situation in your mind. What are you trying to do here? Maybe your team needs to team fight despite having an assassin on the team. So you need to be grouped and ready to flank/dive backline for the 5v5, not just sit in a side lane avoiding the 5v5. Or maybe you have a split push top laner so really you need to be 1-3-1. In that situation you maybe should roam around in jungle a little less and spend more time sitting in a solo lane, ideally a side lane. As an assassin it's really tempting to just spend tons of time hunting kills, but as I said in #1 above, oftentimes just the nature of your character is enough pressure to create space around you. Abuse this and crash waves into towers in side lanes and get good aggressive vision.
One of my biggest points for ALL types of players is remember that there are multiple ways to gain gold in the game and gain advantages. Being proactive and aggressive does not always mean securing lots of kills. Being aggressive does not necessarily mean blood thirsty. Aggression does not have to be tempered by playing smart, it should just be more efficient if you listen to your head and not your heart. For an assassin giving away a lead is generally worse than not snowballing early.
