Lore:

On a faraway world, Polaris was born a member of a race of bestial guardians created to serve as the rulers and protectors of their people. Unlike her brothers Nasus and Renekton, Polaris held a much higher honor. The prophecy holds that the firstborn daughter in the family of the keepers of the Library shall become the next scribe, adding to the vast collection of knowledge, and in turn be blessed with firsthand knowledge of the Cycle of Life and Death. When her brother Renekton was instilled as the Gatekeeper, Polaris received a divine vision that entailed her the specific details on the Cycle of Life and Death. Regaling this news to her people, she was inducted as the Great Library's scribe, the highest honor, and worshiped as a god. Despite her divinity, Polaris was a kind and benevolent ruler, she held her people to the same standard as she did her two brothers.

However, with the power of the gods came a price. Renekton was beginning to succumb to the Butcher's Rage, Polaris, as the Scribe and in-tuned with the Great Library, begun to succumb to it as well. She attempted to purge the evil within Renekton within herself, releasing it into arcane books, where she would seal it in the darkest recesses of the Great Library. Over time however, the "Dark Archives" as they came to be known, became full of the evil intentions of men; all heinous and dark desires and the means to bring suffering and agony, and were filled to the brim. The evil began seeping out, calling to Renekton in the middle of the night, promising Renekton the power to end his suffering by giving into his rage and allowing the evil to penetrate his soul. It eventually caused Renekton to turn against his brother Nasus, and before Nasus could strike him down, Polaris secretly teleported Renekton to Runeterra, to prevent Nasus from befalling the same fate as Renekton. Polaris told Nasus that the Dark Archives were too dangerous to stay upon her world, and thus sealed herself in a spaceship to Runeterra with the archives, in hopes of purifying and containing them.

Ages past as Polaris attempted to purify the Dark Archives, with great success, although the evil had started to penetrate her mind, turning the normally humble and open-minded guardian into a quick tempered narcissist. And on one fateful day, two women Sivir Cassiopeia had opened the door to Polaris' spaceship. The evil immediately escaped into Runeterra, empowering the evil and corrupt with the newfound knowledge of the Dark Archives. Furious, the guardian immediately cursed the one responsible Cassiopeia to the fate that she feared most. The other woman Sivir, Polaris saw deep recesses of goodness within her, and promised that if she were to undo the damage that Cassiopeia released upon Runeterra, and help her track down her brother Renekton, she would reward her with the gift of agelessness or eternal life. Now Polaris fights in the Fields of Justice to bring balance back to Runterra, and to find a cure for Renekton's madness without killing him.

Lore: This is a very good lore. It fits very well with Nasus's, Renekton's, and Sivir's lores, although I would definitely recommend replacing the icons with the actual names. However, you might be contradicting Cassiopeia's lore (?). Otherwise, well written, not going to complain too much about the few grammatical errors.

Passive: This was worded very confusingly. If my interpretation is correct... I don't know about this passive, it kind of forces her to push the lane.

Q: Concept is cool. However... 50% AP per HALF SECOND for 3 SECONDS? uh... that's more scaling than a Kat ult, which is the hardest scaling AP ability in the game currently, as far as I know. Reduce that to 20% or 30% per half second and I think it's OK. You should probably make the blind last for 1 second instead, but still reproc.

W: Uh... okay, but don't you think this is too much packed into one ability? AOE damage and heal and resist buff and resist shred... in one ability. I think it should just be either dmg/heal or resist buff/shred, not both.

E: Scaling too much, otherwise good ability. 30% initial and 60% center maybe? Also this should have a delay with a warning (like Swain's Nevermove perhaps?) so you can't just E right on top of the enemy and instantly snare for 4 (!) seconds and deal tons of damage.

R: Sounds ok (targeted Nasus ult). You should probably just remove the attack buff entirely, or change it to AP scaling completely rather than a weird and overpowered 100% AD + 10% AP thing. AOE is ok.

TL;DR Concept cool, bit overpowered, tweaks would make her a good champion
