Because if it does, it seems like the optimal build. He ults with each attack and the effect on his ult travels to several players at once, so it only takes 5 attacks to kill an entire team if each attack burns 20% health and prevents healing.

#1: No, but Red Buff does. #2: Multiple copies of Morello's stack,(so if Karthus has 3, and ults, each of the people he hits will take X +60% of their max life in damage and can't heal for 5 seconds.) but you cannot have multiple ticks from 1 Morellos.(so you can refresh the duration, but it still only deals 4%/sec)

Ahri also decently uses morello because of the frequency of her skill.
Cho and Karthus are also decent, but the frequency is a little less.

Just as a clarification: After Lucian casts his ability and dashes, he attacks twice:
