TL;DR (Even though it's a twitter post)
- Can never go 3 rounds without an item.
- Later rounds give you an item.

When you see someone at the start have 5-6 items with you having like 2 and then later on killing Dragon and get nothing as well. Its not reassuring that the former won't happen again though.
