Try as I might I can't figure out what the deal is with his shoulder.

The RIoter in charge of his design (can't remember his name) said it is a Mass Accelerator that connects to his pistol and the gun he uses to fire his W to create enough energy to fire his ult. Take a closer look at his pose during the Curtain Call's duration or his ult icon.

Its where he used this hyper pulsor thing that attaches to his gun to create a shoulder mounted weapon for his ult
Actually I think the ionian council did rip his limbs off and left him in the prison, then attached those stuff on him later on.
Have you heard his quotes? That is an extra part of his body, needed to store all of his ego.
