2 cylinders for the bullets.
1 for his Q.
1 for his W.
1 for his R.
1 for his Cigar.
Simple, he didn't have only 2 slugs when he was designed. They just didn't remodel his gun when they reworked him. TF2 has a similar issue with the Loch-n-Load. It has only 2 barrels, but it holds 3 grenades, because the weapon was modeled back when the weapon only had 2 shots.
It was designed horribly. He loads it with 6 shells but every time it fires 2 shells at once and drops a third because it was loose. Every time he dashes he scoops up that dropped shell and quickly loads it back up.
He shoots 3 bullets every shot idk

Well of course there are two barrels so that is still two more... TF card and Cigar?
