Got perma banned on my main, try to log in it says to check my email, email says I broke a rule.

Is that all a reform card is?

Did it come with a banned until date? There will be a code at the end such as "2222", which can tell you what the ban was for. It could be for anything between MMR boosting, extreme verbal abuse, to just a credit card chargeback that you need to handle.

This list is some player-compiled ban codes. It shows 2400 as "impersonating a Rioter", though not all of them are verified, and the codes are for internal tracking and not necessarily going to definitely indicate the reason for the ban (especially according to an unofficial list).

Just submit a Support Ticket and ask about it. Unless you actually know you impersonated a Rioter, then you're out of luck.

Huh thats weird.

Thanks for taking the time to respond and not just straight up insulting me or saying i deserved a ban, i appreciate helping me figure out things that werent clear for me.

If nothing else, 2400 indicates that you have a unique situation and you need to send a ticket to Riot to sort it out.

And to answer your question, no. That is not what a reform card looks like. The reform cards contain chat logs of games. If you ever see it, it is pretty tough to miss.
