How often do you get the pop-up card that someone you reported has been punished ?

For me personally, it's never. Trust me, it's not like I don't report anyone, and it's definitely not like I don't get negativity+harassment in my games. I've seen people post when it works before. Now that the system has been out for a while, I'm curious: are other people's reports significantly more effective than mine?

Bonus Question: When you got your card(s), did you know who it was referring to?

For me I've never gotten it, and trust me I do report people who act up. It's not a big deal to me personally, but it probably just means that their punishments (if any) weren't permaban worthy at that point, which I have no problems with honestly, as if it means that they are willing to reform then it means my job is done as far as they go.

Keep in mind that most toxicity comes from normal people who are simply having a bad day. These people get warnings, so they won't trigger the popup.

I got one for the first time roughly a week ago. Funny thing is I reported a guy from the other team for saying n*gger in the post game lobby. My all chat was muted so I never saw anything he typed in game. I didn't even give it much thought. Walked away from the pc, came back and the message was sitting there. I was shocked. Then it pissed me off. I had a guy saying the same thing in a game on my team about a month ago repeatedly. I reported him and nothing ever happened. Supposedly there's a zero tolerance for this sort of thing but apparently not. No one I've ever reported for trolling me or afking has triggered a pop-up. I'm actually pretty sure my reports are worthless and the fact that I got the message was more of a computerized formality because I also reported the person, than the fact that the system takes my reports serious. Just today I had a guy get mad because I came top lane at 23 minutes into the game to take the turret because nobody was pushing......and he just afked in the base the rest of a game that we were winning up until that point, talking about how he was teaching toxic people like me a lesson. I clicked 3 boxes in his report. He clearly afked in the game. He was negative and he was griefing by just sitting in the base carrying out his little monologue. Pretty sure the whole team reported him. NOTHING. Seriously I'm sure they can spare the money to have a few people who do nothing but review games where people get reported by 3 or more players. I've been trolled HARD since supposedly these messages started and the only one I ever got the message on wasn't even for someone that actually cost me a win.

Never. And I only report for extreme offenses like racist/sexist slurs, sexual harassment, and intentional and admitted feeding.

Heres how it works people

To get that popup that report has to trigger the actual ban. So if you report them and that was the final reprot for them to get banned then youll get the popup. If not then its just tallied onto their account for someone else to report them also till it pops for whoever
