Many people who've gotten punished before trip up on point #2, they think they aren't being toxic, but in truth they are being validly reported for minor toxicity. I suggest you contact support (https://support.riotgames.com) and ask them for your previous chatlogs, post them here so we can give you some advice on what to improve and what to avoid.
It's possible. I've lost my honor Level twice. (Got carried away) and I've gotten back to honor lvl 2 both times.
It is. However, even if you're not additionally banned, if you continue with toxic chat in the mean time (just not enough to get punished), it drastically slows your progress because you're showing you haven't figured it out yet.
You are probably like me. There is a gray zone where you can be validly reported for being "toxic" more or less consistently, but for activity that is not punishment worthy. You can actually work your way down the punishment ladder during this time even though your honor will remain stuck in place
Yes. Took me a week. Am now at checkpoint 1 on my way to level two, though that took a little over a month total.
