I just finished a ranked match with an Azir who went 0/11, at the end of the match I said.. "Azir, wtf". Apparently that warrants a report because I got warned for being an asshole. When I literally said nothing all game other than, "can we group?". I don't understand that.

Having a bad score is not punishable. Troll picking is not punishable. But troll picking and then going 0-11-0 indicates it might be deliberate, in which case he was baiting negative responses in order to be able to report his team.
