This is a report I just wrote for an individual I was placed with in a team builder game.
"1) Queued up as bot lane marksman but went straight to top lane. 2) Did not say a word all game. 3) Has offensive name. 4) left the game."
The amount of times I've been verbally harassed by someone with an offensive name that went AFK.

2 is not necessarily a reportable offense. I frequently go whole games without saying anything.
Well, some report do the same thing anyway.
If someone is doing random things or staying in the foutain, that's Negative Attitude.

The day when this "2) Did not say a word all game." is reportable, I will uninstall this fucking game. I joined game to play not to chat, pings is all you will get from me.
