Are they not self-explanatory?
The only confusing one might be "unskilled player"; that one is a placebo. It's basically just there to soak reports from people who are mad that a player didn't do well in their match, but doesn't actually hurt the receiving player.
If every Rioter explained to you what each report option meant, you'd get a different answer from each. It's entirely a judgment call by the reporter to decide which fits the situation best. Some situations don't seem to fit any (like "hacking"), and for those you'd want to just pick the best you can, and additionally submit a Support Ticket with real evidence to support your claim.
Let's see... going from memory:
Unskilled should be used if you genuinely believe that a player is unskilled. Those reports aren't handled by the Behavioral systems, but instead are used to tweak and tune the matchmaking algorithms.
AFK (leaver, whatever it's called) should be used if a player left the game for any length of time - this includes general lack of participation (dancing in fountain, racing around base, etc).
Verbal Abuse should be used for... well... verbal abuse. Insults, bullying, bashing, berating, belittling, etc.
Offensive Language should be used for genuinely harsh language, most notably hate speech (racist, sexist, or homophobic language, for example).
Intentional Feeding should be pretty self-explanatory. If you feel someone is intentionally feeding the other team, use this report.
Assisting Enemy Team should be used for any other form of griefing - calling out teammates' locations, using Trundle Pillars or Anivia Walls to block allies from escaping/making kills, etc. Keep in mind that it should only be used when those behaviors are intentional - screwing up a wall/pillar placement shouldn't be grounds for a report.
Refusing to Communicate with Team shouldn't really be used. No one's ever been banned explicitly for failure to communicate.
That should cover all of them you need. I'm not looking at the list of options at the moment, so I don't remember what else there is listed there. But these should cover more-or-less every situation you could need a report for.
