It's League - expect the worst out of people. You can't have 10 random players in one game, without a chance of one of them intentionally trying to ruin other peoples days because of it. It's been like this since season 1, and Riot's been unable to deal with trolls since then.

The ban policy is a joke - that's the main flaw. Reform is there only goal - not preventing it from happening. Remember if you troll/flame constantly, your invited to rejoin at anytime/they even lowered the exp required to level 30, due to the massive amount of people required to start over. It's part of the reason League has such a bad reputation with new players. Your actually allowed to troll once every 50 games or so as long as it's not frequent at 0 cost to you.
