Does Elise do 8% or 12% with BORK while in spider form?

Shapeshifters have always been a little confusing when it comes to "melee only" or "ranged only" effects. Does Elise do the 12% of the target's current health in physical damage or does she do the 8 that's ranged? Does it switch when she switches?

Edit: I've tested Phage and it seems to switch it's effectiveness based on her form so it should be the same with BORK I assume?
