Which permabans slip through ban phase, and which team picks them. Enemy gets Viktor top, Irelia mid? You lose. Enemy gets Kaisa bot, Jax top/jungle? You lose. Enemy gets Camille jungle, Akali mid? You lose. etc etc etc
Champ balance is the biggest current issue. And we really need more bans if Riot isn't going to find a new balance team.

These champions are so oppressive and horrible to play against that you have to be completely naive to think that they're balanced. A champion is balanced when they're fun to play as and against. If that condition is met, after their numbers are adjusted, they will naturally have a 50% w/r and be an amazing champion for the game.
These champions only lose when they're put really far behind. If they get their items/levels ON TIME (as in NOT EARLY) they're just so overloaded compared to other champions in the same position.
