They are effectively like old rylai's crystal scepter in how powerful their passives are.

Make them have a window of power with the grievous wounds being on an active.

Morellonomicon should apply the debuff on spell hits too, no more below 40% hp requirement.

Actually, it's below 35% hp threshold... but yeah, most of what OP said is true item 3123 is especially stupid as its a 800g cost item that is a huge middle finger to anyone relying on any kind of healing, but i wouldn't mind that if it's GW application wasn't so stupidly easy and universal...
