Took me some time to find the game this happened. This Sion player builds no defense items like ever. Cait had 50 lethality in items and a item 3036 since the sion did build two small health items but 0 armor. He had over the health needed to have the Lord do the full % increase damage. So this whole gif is just the sion building full damage.
