Do items stack if used by different teammates? If so, should they?

To give an example, let's say that both the support and the top laner buy Frozen heart for teamfights. Do the effects of that stack?

Harder example: Darius and Lucian both but BC and start auto attacking the enemy a Maokai. Is Maokai taking the armor shred of one black Cleaver or both? If he is taking both, should this be allowed when considering how much lethality and %armor pen are getting stacked to kill both squishies and not squishies?

Simplified: Do status effects from different sources stack on top of each other?

BC doesn't(?) but if 2 people but it and hit one person then they'll stack it together, I think

no aura stacks. BC can be stacked by multiple people but it wont go over the cap one of them has

In almost all cases aura effects are unique, and cannot be stacked.

If a champion is near both of the champions with the same aura item, it will only receive one instance of the aura.

For Black Cleaver, the stacks are a debuff on the target. Both champions with Black Cleaver can apply the stacks, but they can't exceed the cap per target.
