Squishies are too tanky, and Tanks deal too much damage
Build
on basically any AD based champion, and become unburstable while very easily bursting a full squishy. This should not be possible. You build this If (for example) you feel the enemy Talon or Orianna is a little too fed. However, as a natural and logical tradeoff, you should not be able to burst then down either. I feel both should take around 70 % hp off the enemy, if equally fed. Likewise, a fulltank Malph initiation should make you vulnerable for the follow-up, not almost kill you if you are full squishy.
These items are the root of almost all balance problems of the game. As burst doesn't work, sustained damage is the only reliable way to go. So autoattacking champions are super strong. ADCs are of course really good right now, melee AA'ers like Yi, Shyvana, Udyr, Tryndamere and so on is having a field day - even after nerfs. Next on the FOTM list is Volibear, who is extremely tanky with tons of damage. The Tank Akali and Tank Yasuo builds just shows how strong hybrid items are.
The solution is obviously to nerf hybrid items (and to be fair, Riot are slowly doing this), and i think a good idea is to buff Support shields (both items and kit-wise), so it at least takes 2 people to make a high-damage dealer "unburstable", and buffing the Support role at the same time.
TL:DR restore the balance of burst > sustained dmg, sustained dmg > tankyness and tankyness>burst. Right now, sustained dmg > all.
PS:
is a problem because his burst comes before you can build a full item. Nerf incoming.
?? Basically, a tradeoff for building defensive.
because everyone wants to take 180 damage every two hits from one item...
