Need some advice

Hey guys I just had a quick question. I play a lot of mid lane and I've been analyzing my gameplay which got me thinking about my early game play-style. I win lane a lot, but I know I'm no where near great at the game so don't get the idea that I'm trying to brag or anything. It's just easier for me to 1v1 people and attempt to outplay them in laning phase. But this is about all I do, I never transition well into the late game. I tense up and choke in teamfights and I always tunnel-vision or whatever its called when you don't look at the teamfight as a whole. I dodge skillshots while doing damage as well when playing against an Ahri or Veigar for example, but I don't do any of this in teamfights. So I guess what I'm asking is how do I fix this? I really am trying but my bad habits always get the best of me in teamfights and the late game. So should I just play late game mages that do a ton of AoE damage in teamfights like Annie or Viktor? Play who are you are comfortable with. Every time from now on you get 1 or 2 kills by say... 5-10 minutes, gank somebody in another lane or invade their jungle. Just do something that could possibly help someone else on your team. Learning to get you, the easiest 1/5 of your team, fed is cake for a mid laner. Being a central pillar that supports your team and can snowball 2/5, 3/5 of your team wins games.

Watch your map at all times. If something happens at the top of your screen and you weren't at least aware that it could happen then you aren't watching your map enough. If you ever think, "Ugh, that should have been warded." then keep it warded. Try out boots of mobi, they help a lot when you learn to get ahead early.

Learn how lanes flow. Learn whether a lane is going to go towards their turret or yours or stay still. Those are all predictions you can make easily by looking at your map. If it is going toward your side then what are you waiting for? You should already be there.

That is my advice, sorry if it wasn't for as late game as you were looking for. If your issue is teamfighting specifically then try a few aram matches, that is basically just teamfighting all game.

Stop panicing in teamfights and understand your what your role is in them. That relies on which specific champions you play. Be careful about your positioning in teamfights because one mistake and you're probably dead.

Instead of just winning the lane try also roaming to other lanes and by that snowball the whole game, not just yourself.

Dont stop farming, understand what you're supposed to do as the champion you play in just about every situation (teamfight, sieging, defense, contesting on objectives) and this should help you being useful also in later teamfights.

Do not do desperate moves or try to fight in risky situations. Ask yourself twice before going in, because a lot of the carrying's burden is on you, and if you screw up it's much more dangerous to your game, than if your top laner or support died. (in some situations you're even more important than adc).

It's all about position. Doesn't matter if you're behind or ahead, late game it comes down to what you can put yourself in. Never choke. Never be scared. Never do anything that kill yourself or the team. Focus on dealing damage to the frontline becasue they are the ones after you.
