PTA needs a buff and/or Conqueror needs to be nerfed.

PTA: Either make PTA be able to proc on multiple targets but Runnan's doesn't count towards proccing stacks. Or make PTA have the vulnerability constantly refreshed whenever the champion takes damage. Or increase the vulnerability percentage to 12%-25% (based on level).

Conqueror: Either reduce the amount of true damage Conqueror can deal to 10% and remove the bonus AD. Or change Conqueror to 4 seconds in combat with champions, the next attack will proc Conqueror. Or remove the melee proc refresh and add a cooldown of 6 seconds after Conqueror has ended.
